Each client is assigned a dedicated nurse who conducts weekly in-person visits to monitor health, identify early changes, and intervene when needed. This continuity allows us to catch issues early, reduce avoidable hospitalizations, and help patients stay in the comfort of their own homes for as long as possible.
Our interdisciplinary team includes not only expert nurses, but also occupational therapists, physical therapists, and speech therapists. This ensures that every client receives comprehensive, well-rounded care at home.
We act as an extension of your team, promptly communicating any changes in condition and supporting patients and caregivers in following through with recommended treatments or next steps.
