Step 1: Book Your Free Consultation
Start with a complimentary 30-minute consultation. We’ll get to know your needs, answer your questions, and discuss how Trusted Path can support you and your loved one.
Step 2: Personalized Care Planning
After the consultation, we’ll schedule your first in-person visit. During the first few visits, your dedicated nurse will assess your situation, review healthcare records, and identify ways to improve communication across your medical team. We’ll also introduce helpful community resources and explore any insurance benefits that could support your care—now or in the future.
Step 3: Ongoing Support and Visits
Following the initial assessment period, we’ll tailor a care plan that meets your evolving needs. All visits are conducted in person whenever possible, with telehealth options available for urgent concerns when needed.
Step 4: Easy, Automated Payments
Weekly services are billed automatically on a weekly basis to keep things simple and stress-free. No surprises, just consistent, reliable care.
